Those without letters of referral

1. Reception

Please come to the No. 1 ‘General Reception Desk’.
Fill out the medical application form, and submit your health insurance card.

*Those who don’t have letters of referal from local medical institutions will be charged ‘selected medical expenses’ apart from the normal initial consultation fee.
Your waiting time may be longer, and you may not be consulted at the clinical department you want.

* If you have them, please bring your ‘health insurance card’, ‘elderly insurance certificate’ and ‘publicly-funded health care certificate' when you come to the hospital. If they cannot be confirmed, you may be liable for all medical expenses incurred.

 

2. Receiving a Medical Card

After registering, you will be given a medical card and a ‘reception slip’.
Please come to the outpatient reception desk with the same number that is on the reception slip.

① What is a reception slip?
The reception slip is for confirming that the patient has arrived, and also for calling the patient with the number written on the slip.
It is issued to all patients at the ‘No. 5 return reception machine’ or at the ‘outpatient reception desk’

② Please come to the ‘outpatient reception desk’ with the corresponding number.

③ Please come to the ‘No 3 general reception desk’ or ‘outpatient reception desk’ nearby.

 

3. Outpatient Reception ・Medical Examination

A ‘basic card’ will be issued in exchange for the reception slip at the outpatient reception desk.

① The ‘basic card’ is for guiding places that examinations etc. will be held in the next step.

② The ‘basic card’ is for guiding places that examinations etc. will be held in the next step. ③ Please show your ‘basic card” at the next reception desk in order to confirm your arrival.

③ After your medical examination, please come to the ‘general reception desk’ as you will be given the basic card stub.

* For the sake of privacy, you will not be called by name, but instead by the number written on your ‘reception slip’ and ‘basic card'.

*The application numbers will be displayed on the digital screen in every examination room. When your appointment No. is displayed, enter the outpatient lounge.

*Please wait at the outpatient lounge. The doctor, nurse, or doctor’s clerk will lead you to the examination room.

 

4. Payment

After examination, you will be given an accounting stub.
When accounting is ready, your number will be displayed on the digital screen at the general reception desk. Please pay at the No. 3general reception desk or by the No. 6 ATM.

 

5. Receiving Medication

Hand over the prescription given after the examination to an out-of-hospital insurance pharmacy and receive your medication. At the ‘out-of-hospital prescription’ corner inside the front door, we can reduce your waiting time at the pharmacy by faxing your prescription contents to the pharmacy of your choice. Also, if you're unfamiliar with the closest pharmacy to your home, we can locate it for you at the reception desk.

Notes

* As the ‘hospital card’ is available for all clinical departments and necessary for reception and payment, please bring it when you come to this hospital. Reissuing cards requires an additional fee.

* To ensure medical safety, our patients' facial photos are registered in our electronic medical records. Thank you for your cooperation. Patients who have already had their photo taken in the past year can skip this step.
